Show moreType 2 diabetes is a public health challenge. Researchers at the center of disease control and prevention (CDC), report more than 34 million Americans have diabetes (about 1 in 10) (CDC, 2019). Uncontrolled diabetes over a long period of time can result in end-organ damage. End results of poor glycemic control can result in diabetic retinopathy, neuropathy, kidney disease and kidney failure (Sacks, 2017). In type 2 diabetes (T2D) cells do not respond normally to insulin, a condition known as insulin resistance. These results set the stage for pre-diabetes and type 2 diabetes. Type 2 diabetes often develops in people over age 45, but more recently children, teens, and young adults are also developing type 2 diabetes. Type 2 diabetes in children are rising due to obesity rising. According to the CDC, more than 75 % children with type 2 diabetes have family members who also have it (CDC, 2019). With this being said, many evidence based research studies reveal how diabetes self- management is influenced by one’s confidence and willingness to manage diabetes. The patients desire to enhance their knowledge, skills and confidence is referred as “Patient Activation.” Increasing patient activation will help people manage their diabetes more effectively and be active in self-management behaviors (Regeer, Van Empelen, & Bilo, 2021).

Show moreBackground: Worldwide, Colorectal cancer (CRC) is the third leading cause of death (Bevan & Rutter, 2018). In the United States of America, Colorectal cancer is the second leading cause of death related to cancer (Green et al., 2019). The Center for Disease Control and Prevention (CDC) reports that millions of people are not screening as recommended and thus missing the chance to prevent or detect early CRC when treatment could often lead to a cure (CDC, 2016). CRC is a highly valued preventive service, but sadly underused. Purpose: The project goal was to evaluate the Colorectal screening program in one of the Florida Community Health Centers (FCHC) to determine if the center meets the National goal of 80% within the past 10 months, and to provide recommendations for future screening practices. Methods: This project was conducted using retrospective chart reviews to calculate the CRC screening rates from July to September of 2019. Recommendations were made using the best available evidence. Results: The clinical analyses of the CRC screening of the FCHC revealed only 60% of patients were screened by September 2019. As per the survey conducted among the providers showed 100% awareness on all aspects except there is no designated staff that follows the CRC screening. Implications: The Florida Community Health Center (FCHC), failed to reach the National goal of 80% and the Health 2020 people goal of 70%. To increase the CRC screening rates, it is suggested that a new project champion be assigned to ensure the program’s success.

The use of probiotics has been widely explored in topics pertaining to gut health, however creating a more balanced flora of bacteria within the vagina could also lead to advancements in women’s health. This doctoral project was designed and implemented to increase the knowledge of providers and patients about the long-term use of probiotics for vaginal health while decreasing symptomatology at a women’s and family practice in South Florida.

Show moreHuman papillomavirus (HPV is the most common sexually transmitted infection(STI) in the United States, with an estimated 80% of sexually active people contracting it at some point. HPV can cause cervical, vaginal, and vulvar cancers in females, as well as anal cancer and genital warts in males. However national data demonstrates less than 50% of females are fully vaccinated. This quality improvement project aimed to bridge the gap in young women who did not receive the vaccine during childhood as well as increase knowledge about HPV, transmission, vaccination, and testing. The interventions used to facilitate the education included an HPV pamphlet and direct education with the trusted provider using motivational interviewing techniques. Pre and post tests were given to 48 participants that assessed HPV knowledge and intent to vaccinate. Participants included females < 27 years old with thirty-six participants who fully completed all aspects of the QI project at an OBGYN private practice. The intent to vaccinate in the pretest was 8% and posttest was 67%, with a percent increase of 700%. The pretest HPV knowledge average test score was 72% with an average post test score of 91%. The results of the project add to the body of evidence-based practice knowledge and clearly demonstrate the effectiveness of education with a strong provider recommendation in increasing vaccination uptake. The methods and modalities used for this project could easily be implemented in any practice following this particular patient population such as university student health systems or family practices.

The purpose of this DNP project was to analyze providers’ awareness and beliefs regarding antibiotic use and resistance and to evaluate the effect of an antibiotic stewardship program on urgent care providers’ antibiotic prescribing for acute bronchitis. The overall goal is to decrease antibiotic resistant infections by reducing inappropriate antibiotic prescribing for a viral infection. To achieve this goal, a quality improvement initiative was conducted with five participating urgent care providers. Pre-RAMP intervention there was an average of 64% of over prescribing antibiotic for acute bronchitis, post-RAMP intervention was 18% which was a 71.8% reduction of over prescribing for acute bronchitis. With continuation of the RAMP intervention, at a greater scale, it will produce a sample size with enough data to fully support the cause for awareness of antibiotic prescribing for safe and quality patient care.

The MA for the purpose of this paper and project MA is defined as Proportion of Days Covered (PDC) of ≥ 80 % when patients fill their prescriptions on time as ordered by their Primary Healthcare Provider (PCP). There are devasting effects of medication nonadherence, which causes preventable deaths, hospital admission, injuries, and rising healthcare costs. Therefore, MA on certain maintenance medications for blood pressure, diabetes and cholesterol are included as a patient care quality measure in the Healthcare Effectiveness Data and Information Set (HEDIS) by the Centers for Medicare and Medicaid Services (CMS) among Medicare Advantage beneficences. PCPs are accountable and responsible for their patients for adherence with those medications. Providers or practices that achieve target scores in HEDIS on MA financially rewarded with bonuses from the health plans that bring extra revenue. This doctoral project and paper intended to help providers and patients to address MA by implementing the Integrated Multistep Patient-Centered Program (IMPCP) in primary care to find out the effects on HEDIS scores and Medicare Star Rating (MSR). IMPCP was developed to address MA in the local primary care Managed Service Organizations (MSO) in South Florida. This design of this project was quasi - experimental with secondary analysis. Descriptive statistics were used as a method to manage data. In conclusion, the IMPCP improved HEDIS and MSR scores in the local MSO. This paper includes the description IMPCP, results, evaluation, critique, and plan for dissemination.

The rise in the prevalence of obesity has been accompanied by a rise in the prevalence of diabetes and nearly 90% of newly diagnosed patients were overweight (Rossouw, 2015). Most guidelines recommend promoting weight loss among overweight or obese individuals by reducing energy intake together with a healthy eating pattern that focuses on a diet composed of whole or unprocessed foods combined with physical activity and ongoing support (Forouhi et al., 2018). Research also show that behavioral interventions for obesity involving face to face meetings in group sessions typically result in weight loss. The purpose of this quality improvement project is to initiate a practice improvement project through information dissemination using visual displays for healthy eating to improve knowledge, attitudes, and perceptions/practices toward proper nutrition that will ultimately support weight loss and diabetes control. The project was implemented at a federally qualified health center that caters to the underserved population of Okeechobee, FL. The program was led by a prospective doctoral candidate and involved group-based sessions, interactive teaching focused on dietary modification, and offered social networking and social support. Outcomes included improvement in anthropometric measurements, significant Knowledge, Attitude, and Perception (KAP) survey difference scores, and the ability to create or demonstrate a meal plan that suits preferences and reflects individual total daily caloric requirement.

Show moreHaving high cholesterol levels raises the risk for heart disease, the leading cause of death in the United States. High cholesterol is considered a silent preventable condition. Uncontrolled cholesterol levels increase health disparities experienced by patients of low-socioeconomic status. The purpose of this quality improvement (QI) project is to determine the effect of an evidence-based cholesterol education program in enhancing hyperlipidemia management strategies among Care Coordinators at a Federally Qualified Health Center (FQHC). The overall goal of the project is to improve patient’s cholesterol levels over a 3-month period. This project supports the implementation of enhanced hyperlipidemia education for Care Coordinators based on a literature review and national hyperlipidemia guidelines. The Lipids, Learning and Lifestyle (LLL) program was created by the project leader and delivered in a train-the-trainer approach to the Care Coordinators at the FQHC. The project leader administered a modified Hypertension Knowledge-Level Scale (HK-LS) questionnaire to Care Coordinators before and after the training sessions, and score results were compared. A total of 10 patients were given the enhanced education by Care Coordinators, in individual sessions attended by the project leader. The patient’s pre and post-intervention cholesterol levels were compared to determine the impact of the educational program. Results from the modified HK-LS did show a statistically significant change in mean scores from pre intervention to post-intervention, revealing an overall improvement in knowledge about hyperlipidemia management. Review of the patient data showed there was no statistically significant improvement in Total Cholesterol (t(10)=1.60, p=.14), HDL (t(10)=-.69, p=.51), Triglycerides (t(10)=1.71, p=.12) or LDL (t(10)=.23, p=.82), from pre-intervention to post-intervention. Though there was no statistically significant improvement in overall Cholesterol levels for the patient participants, there was a statistically significant improvement in Care Coordinator knowledge about hyperlipidemia management.

Show moreThe goal of this quality improvement project was to increase medication adherence in patients post allogeneic hematopoietic stem cell transplant (allo-HSCT) following hospital discharge. The aim was to evaluate the impact of implementing an individualized post-transplant education intervention, grounded in Roach’s human caring attributes, on medication regimen adherence post-transplant (allo-HSCT) following hospital discharge. The intervention included use of a pill box and medication calendar (PPM). Patient-Nurse interactions were guided by conscience and occurred within an environment of trust, respect, and compassion in which the nurse connected in full presence to the patient’s situation and demonstrated skills and knowledge (competence). A prospective, single arm, instructional intervention design was used to compare medication regimen adherence pre- and post- intervention. The effectiveness of the educational approach was evaluated using the Morisky Medication Adherence Scale-8 (MMAS-8). Participants included all patients who underwent allogeneic stem cell transplantation and discharged from the project facility between June 15th, 2021 and September 15th, 2021 (n=10). Prior to the PPM intervention, 20% of patients fell into the high medication adherence category, 40% were in medium adherence, and 40% in low medication adherence category. Four to five weeks after the PPM intervention, 70% were found to be in high adherence category and 30% in the medium adherence category. A paired sample t-test conducted on mean MMAS-8 score Pre- and Post- PPM intervention was found to be statistically significant (Mean 6.725 Versus 7.675, p=0.034). Early implementation of the PPM intervention involving individualized post-transplant instruction, grounded in caring science, was beneficial for post-transplant medication adherence in allo-HSCT patients following hospital discharge. Future research studies are needed to confirm the effectiveness and generalization of the PPM intervention.

Show moreClinical question: In parents with children 3-5 years of age with reported behavioral disorder problem behaviors (P), how does implementing a mindful reading strategy with their children, (I) compared to traditional reading or no reading (C) effect parents’ perception of problem behaviors and parent-child relationship (O) within 8 weeks (T)? This quality improvement DNP project examined the effectiveness of an online video parent training program, Mindful Reading Strategy for Parents, created based on the recommendations of Susan Hendler Lederer, PhD (Lederer, 2018). The theory of Nursing as Caring by Boykin & Schoenhofer was used to ground this project in caring (Boykin & Schoenhofer, 2015; Boykin & Schoenhofer, 2019). A convenience sample of three English speaking participants with children aged 4 and 5 with parental reported behavioral disorder problem behaviors were recruited from an outpatient clinic and preschool. They were assessed prior to the video training and 8- weeks after using the Parent Perception Survey and Pre- and Post- Parent Survey. Parents reported improved parental perception of child's behavior, perception of at least one behavioral disorder problem behavior in their child, and perception of parent- child relationship. No changes in child's psychotropic medication regimen post intervention were reported and 66% of parents read to their child at least 4 times per week during intervention. In this project, the Mindful Reading Strategy for Parents training program can have a positive effect on parents’ perception of problem behaviors and parent-child relationship. Implications include increasing parent and provider education of benefits of early intervention of mindfulness embedded reading strategies for children. Based on participant feedback, shortening the video training program may improve parent participation.

Methods: Nine recruited participants consented to participate in the program. Each participant was given a Diabetes Empowerment Scale before an education session as a pretest. In addition, all nine participants were provided with a curated educational packet and teachings in their primary language during the session. After the education, the participant completed the Diabetes Empowerment Scale as a posttest to evaluate the participant’s psychosocial self-efficacy toward diabetes.

Show moreSubstance abuse, specifically opioid abuse, continues to be one of the largest epidemics in the United States. Death caused by overdose has seen a steady increase over the past few decades, and despite public health initiatives such as clean needle exchange programs, methadone clinics, and Narcan, there continues to be a concerning rise in the number of drug usage and death secondary to drug overdose in the young adult. Recently, Narcan became approved for over-the-counter usage to combat the increased incidents of overdose. Scholarly evidence shows that opioid use disorder is most effectively treated with a combination of medication and psychotherapy, particularly Cognitive Behavioral Therapy (CBT). Although Medication-Assisted Therapy (MAT) remains the gold standard in the treatment of opioid use disorder, the evidence and literature presented throughout this project will document how much more effective treatment is when patients are engaging in psychotherapy. The evidence shows that patients have a reduction in cravings, there is a reduction in relapse, and in many cases the patients can maintain long-term sobriety utilizing the tools learned in psychotherapy sessions. This project is a quality improvement project, in which I assessed the current processes in place to assess current treatment of patients seeking initial psychiatric evaluation, as well as of those engaging in follow up visits for the treatment of OUD. From there, I realized that my project site did not have a current process in place unique to patients being seen for OUD. I developed a questionnaire, based on similarities between our patients seen for OUD, that would be used with patients to establish a baseline and used to gauge effectiveness of treatment at increments. Ten patients were followed from May to October to generate these findings that will be presented. In the end, this project has provided a promising conclusion: combined therapy with MAT and psychotherapy for treatment of OUD is effective in optimizing sobriety.

Show moreA lack of knowledge and involvement in grassroots legislative advocacy among nursing professionals adversely affects nurses’ ability to effectively tackle issues impeding the safe and effective delivery of healthcare. The role of primary care nurses in improving health outcomes includes developing substantial legislative relationships that constructively introduce nurserelated issues that are not adequately recognized or understood by legislators. This DNP quality improvement project sought to assess the effectiveness of a four (4) week legislative advocacy training program on the knowledge of, confidence in, and participation in the legislative advocacy process and local legislative outreach among nurse members of the Florida Nurses Association. Following the four (4) week intervention period, self-reported knowledge and confidence improved significantly evidenced by a reported 77.8% increase in advocacy knowledge and an 81.5% increase in advocacy skills. More than 80% of participants documented confidence in their ability to participate in legislative outreach with a 44% increase in contacting a legislator by email or mail, a 7.4% increase in legislative outreach by telephone, and a 7.4% increase with in-person visits. The results of this study suggest that increasing advocacy training, by incorporating frequent, repetitive sessions over a short period of time, is an effective approach to improving legislative outreach among nurse professionals.

Approximately 7 to 12 % women are affected by prenatal depression (Sidebottom et al., 2021). African American women experience more stressors during the prenatal period which places them at a higher risk for depression than other racial groups (Liu et al., 2016). Early screening can help identify and treat prenatal depression which may reduce the risk of postpartum depression. The purpose of this quality improvement project (QI) was to screen African American pregnant women, using the Edinburgh Postnatal Depression Scale (EPDS), to determine if screening for depression prenatally can initiate early treatment and referral. Method: All pregnant women who received prenatal care at a women’s health clinic were screened, using the EPDS, for depression prenatally. Relationships and differences between mother’s demographic variables and their EPDS scores were explored using the Pearson Product Moment Correlation and Analysis of Variance (ANOVA). A one-time follow-up telephone interview was conducted, for education and support, 3-4 weeks post completion of the EPDS.

Show moreOpen-heart surgery is one of the most frequently performed surgeries in the United Sates. Despite advances in techniques, there is still a significant number of complications leading to readmission. As of 2021, the national pooled 30-day readmission rate was 12.9% (95% CI: 11.3-14.4%). Various studies report rates of 30-day readmission from 8% to 21%, thus making open-heart surgery a target to achieve value-oriented health care milestones by policymakers. A program evaluation was conducted via chart review to compare the rate of readmission of a local cardiothoracic program to the national average. The program focuses on early follow-up post discharge and direct communication and access to the team (surgeon and advanced practice providers). A total of 98 patient charts were reviewed and the rate of readmission was calculated to be 6.1%. There was also a 3.1% rate of emergency room visit without readmission noted. Due to the significant different between these percentages, it is recommended that the team shares their processes to develop a clear protocol for other teams in the area to adopt into their practice.

AAD is a common yet underreported side effect of antibiotic therapy that often leads to interrupted or premature discontinuation of antibiotic treatment that may result in a possible increase in long-term complications and other health implications related to AAD. The use of probiotics to prevent AAD and their general positive impact on gut and overall health are well-studied and documented. Providers are generally aware of the recommended best practice regarding probiotic prescribing to prevent AAD. However, many providers fail to implement the recommendations into practice. A common theme among providers is a lack of knowledge as to which probiotics to recommend given the vast choices of possible probiotics available, the length of treatment, or the most favorable time frame for probiotic therapy. To reduce AAD occurrence and improve patient adherence with antibiotic therapy, this doctoral project aimed to improve provider probiotic knowledge and prescribing practices in a South Florida urgent care and a primary care setting.

The terms are often used interchangeably to describe a feeling of deep sorrow felt after the loss of a loved one. Often it can be difficult for individuals to cope with the loss of a loved one, and it is estimated that 20% of bereaved individuals will experience complicated grief (NAMI, 2021). Complicated grief, often referred to as a prolonged grief disorder, can be defined in several ways. Today, the most widely accepted definition of complicated grief is grief that persists for longer than one year and significantly interrupts an individuals’ ability to complete daily functions (Cleveland Clinic, 2022). Each year, there are roughly 2.6 million deaths in the US alone, leaving behind approximately 13 million individuals (about twice the population of Arizona) to grieve (U.S. Census, 2021). A study conducted in 2019, surveyed 1,084 US adults and found that 32% had experienced grief following the death of a loved one in the past three years (The Grief Experience, 2019). Furthermore, of U.S. adults over the age of 65, 71% reported experiencing bereavement in the previous 30 months (Williams & Sawyer, 2017).

Show moreDespite aggressive efforts to help people achieve target blood pressure goals, currently in the US, only 52% of patients with hypertension have achieved blood pressure goals as established by JNC 8 (Low et al., 2014). Hypertension correlates with a greater risk of myocardial infraction, heart failure, stroke, and kidney disease (Low et al., 2014). Early detection and intervention is essential to reduce mortality and morbidity (Beune et al., 2014). Target BP is a national initiative formed by the American Heart Association (AHA) and the American Medical Association (AMA) in response to the rising incidence of uncontrolled blood pressure (BP). The program established protocols including teaching patients how to measure blood pressure accurately as well as partnering with families, patients, and communities to promote self-management goals in hypertension. The Target BP program was implemented by the staff at one primary care practice in South Florida to improve blood pressure in their patient population. This health center serves hundreds of thousands of patients in the South Florida area to meet the needs of primary care in the community. The intention of this doctoral project was conduct a program evaluation of Target BP within this community health center in order to understand if the program should be adopted, adapted, or abandoned. All 114 patients enrolled in the Target BP program at the community health center where evaluated for improvement in hypertension during a four-month period. Twenty-five patients had blood pressures collected for all four months. The community health center exceeded their goal of 10% decrease in in the patients with uncontrolled hypertension within the organization. In total, 47 (41%) patients showed improvement in blood pressure to normal limits according to JNC8 standards over the 4 months. Program interventions were implemented per protocol when patient’s blood pressures were out of range. Staff nurses and healthcare providers provided interventions to address diet, exercise, monitoring blood pressure on logs, as well as taking medication for hypertension to encourage achievement of Target BP goals.
