Show moreThe present study investigates the affectual interaction of three highly competent therapists using different theoretical approaches with a shared male client and female client in a professionally recorded video series. The interactions of clients and therapists in a total sample of six psychotherapy sessions were coded using the twenty code version of Gottman, Woodin, and Coan’s (1998) Specific Affect Coding System. Coded data were analyzed using Kruskal-Wallis tests which found no significant differences between the mean ranks of therapists’ codes between therapists. Significant effects among some affective behaviors were noted in therapists’ codes when compared by client. Coded data were also used to create mathematical models using ordinary differential equations for each of the six sessions. Kruskal-Wallis tests did not reveal significant effects in the mean ranks of the parameters of the mathematical models, and visual similarities and differences of these models were discussed. Additional analyses were conducted to examine clients’ affective behaviors as well and significant effects were revealed in the Kruskal-Wallis tests amongst many coded behaviors. The results support the presence of common factors and similarities in the therapeutic relationship among different theoretical approaches. The findings also add to the growing body of literature dedicated to the use of observational coding and dynamic nonlinear modeling in psychotherapy research. The implications for psychotherapy practice, education, and research are discussed.

Show moreIn order to prepare counselor-trainees to practice competently and maximize client outcomes, counselor-trainees must develop a broader understanding and explanation, called a case conceptualization, of their clients presenting problems, personality, and systemic dynamics. There is a limited body of research that examines the effects of training on the ability for counselors to develop an effective case conceptualization. This is the first study to train counselors to develop competence in formulating an integrative case conceptualization. This quasi-experimental study aimed to determine whether an intervention had an effect on the 85 counselors in training that participated in a 2-hour training. Data was analyzed using paired sample t-tests, an independent t-test and correlation methods. Participants’ CCEF scores significantly improved from the training (t = -25.970, df = 84, ES = 3.67, p < 0.001). Participants’ CFCCM scores significantly improved as well (t = -21.693, df = 84, ES = 2.78, p < 0.001). Participants’ scores also significantly improved with their perceptions about case conceptualization (t = -30.828, df =84, ES = 5.47 p < 0.001), for items 1-5. Item 6 resulted in values of (t = -13.295, df = 84, ES = 2.14, p < 0.001). The level of cognitive complexity as measured by the CCQ did not have a significant effect on the quality of the counselors in training case conceptualizations. Compared to the CFCCM, the CCEF is a much shorter and easier instrument for evaluating a case conceptualization. Overall, results of this study provide support that counselors in training can improve their quality of case conceptualization skills in as little as 2 hours. This can aid in understanding and explaining their client’s situation better, which may significantly improve the treatment process and client.

Show moreThe purpose of this quasi-experimental study was to investigate the effects of equine-assisted psychotherapy (EAP) on mindfulness, self-reflection, insight, and psychological well-being in an adult veteran population with mental health concerns. Specific aims were (1) to determine the effect of EAP on mindfulness, self-reflection, insight, and psychological well-being in veterans with psychiatric diagnoses or mental health concerns; and (2) to describe the relationship of the sociodemographic characteristics (age, ethnicity, gender, education level, income, and deployment history) to mindfulness, self-reflection, insight, and psychological well-being of adult veterans with mental health concerns engaged in EAP. Smith’s (1999) theory of unitary caring provided the guiding theoretical and conceptual framework for the study. A convenience sampling design was used to recruit 18 participants from a South Florida therapeutic riding center and an online veterans’ forum. The sample consisted of adult veterans ages 18 years and older who had mental health concerns and/or diagnoses. Assignment to the EAP group (n=9) was determined by the therapy center director based on session days and times and participant availability. The comparison group (n=9) received their treatment as usual.

Coded data were analyzed using Kruskal-Wallis and Mann-Whitney U tests which found significant differences between clients who dropout and the types of questions being asked in session (HSS). There were also significant differences between clients who dropout and the therapist and client SPAFF scores, SPAFF and HSS scores on the WAI and RRI, as well as the quality of questions being asked (HSS) over time (from initial session to fourth session). Coded data for differences between clients who dropout and the therapist and client assessment of the quality of the working alliance and real relationship were assessed using Kruskal-Wallis and Mann-Whitney U tests and found no significant differences. Analysis of the results support the presence of therapist factors on the therapeutic relationship and client premature termination. These findings can also be added to the literature regarding the outcomes of the therapeutic relationship on client premature termination. The implications for psychotherapy practice, education, and research are discussed.

A sample of 63 full counseling sessions (21 first sessions with clients who return for four sessions, 21 first sessions for clients who terminate therapy prior to four sessions, and 21 fourth sessions) were collected from a university counseling center in South Florida. Clients and clinicians also completed self report evaluations of the therapeutic relationship that were also included in this study (Working Alliance Inventory – Short Form, and the Real Relationship Inventory). A series of multivariate analysis of the variance (MANOVA) tests were performed to assess for significant differences in Turning System behavior between return and dropout groups in the first sessions of therapy, as well as for significant differences between return groups’ first and fourth sessions. Correlation analyses were run for client and therapist self report data and Turning System codes. Overall, the Turning System codes did not predict attrition from therapy; however, significant effects were found for specific behaviors in the return versus dropout comparisons, including client’s uses of negative bids, high level questions, and preoccupied away responses. The Turning System also did not predict clear differences between behaviors in ongoing therapy, though significant effects were again found for individual behaviors enacted by both clients and therapists. Significant negative correlations were also found for return group clients between specific behaviors and ratings of the therapeutic alliance, such as high level questions and preoccupied away responses. Individual behaviors in the therapist return group, such as high level questions, negative bids, and interruptions, correlated negatively with ratings of the therapeutic alliance. The results of this study are presented in an effort to synthesize the data into a narrative for developing effective therapeutic relationships and guidance for future research.

We describe the emotional state of both the therapist and client with coupled, first order, nonlinear ordinary differential equations (ODE's). The rate of change of the emotional state of the therapist and client is proportional to their previous state, their uninfluenced state when alone, and an influence function which depends on the state of the other person. We formulated influence functions based on the research literature on psychotherapy and the therapeutic alliance. We then determined the critical points from the intersection of the nullclines and used a numerical ODE solver (Matlab ODE113) to compute the trajectories from different initial conditions. ... The results validate this prototypical approach to psychotherapy ; we have shown that human interaction (in the context of psychotherapy) can be quantified and modeled using differential equations.

In addition to building a more solid foundation of literature in this field, this study investigates at-risk teenage female residents' perceptions of educational experiences within a residential treatment environment. The relationship between formal, informal, and non-informal experiences, preferred learning strategies, and success patterns were explored. This phenomenological study utilized a lens of critical theory to understand the meaning of girls' educational experiences at a residential treatment center. The design includes studying 16 girls' formal, non-formal, and informal educational experiences as well as utilizing the Assessing the Learning Strategies of AdultS (ATLAS) tool to assess learning strategy preference. Results of the study determined student preceptions regarding various educational environments and learning strategies at a residential treatment center. Analysis of interviews and data collected led to many significant findings. Additionally, this study enhances knowledge in the field of residential treatment and has implications for those seeking residential treatment, working in residential treatment, and educators. Research in this field often focuses on outcome and effectiveness literature. This study provides insightful information incorporating the voice of students into the research.

Lifestyle constructs have been seen as predictors of substance abuse related behaviors, including future drug related difficulties. Dysfunctional family environments have been seen as predictors of poor mental health outcomes in family members. Despite the high rates of co-morbidity in alcohol and drug dependent individuals, parental lifestyle, attachment style, and the overall influence of family environment on the psychological traits of substance dependent individuals had not been previously examined. This study examined a group of parents and their adult children (actively in substance abuse treatment, with diagnoses of substance dependence) who participated in a weeklong family education program at an inpatient treatment center. Direct effects were found between attachment style, lifestyle and patient traits, as well as several significant total effects (combined direct effect and the effect of overall family environment). Specific indirect effects were also found from individual family environment mediators, including moral religious emphasis and control, on the relationship between independent and dependent variables. An analysis of the results is given, along with a discussion of clinical implications and directions for future research.

Show moreA quasi-experimental, between groups design was used to evaluate differences in emotional behavior, as measured by the Specific Affect Coding System (SPAFF), between professional therapists and their clients and a master therapist and his client. This coding system also was used to determine how emotional behavior shown by a master therapist changes over the course of six psychotherapy sessions. The research team recorded counseling sessions at a university counseling center in the southeastern United States and coded this video data using SPAFF in real time. Data were analyzed quantitatively to determine whether significant differences in SPAFF codes exist between the master therapist, professional therapists at the university counseling center, and their respective clients. Results indicated that the master therapist showed significantly more neutral and less negative affect than his counterparts at the university in both sessions one and four. The master therapist’s client showed significantly more neutral affect and less negative in session one and significantly more positive affect and less negative affect in session four.

Show moreThe present study evaluated affective, relational, and emotional behaviors associated with symptom reduction and clinical outcomes in psychotherapy. It relied on 36 therapist-client dyads and assessed client symptoms using the Counseling Center Assessment of Psychological Symptoms (CCAPS-34) as the progress monitoring and outcome assessment tool used by university counseling centers before every psychotherapy session. The research examined the first administration scores and the last administration scores of the CCCAPS to determine the level of clinical improvement. The researcher used the revised Specific Affect Coding System (SPAFF) to code the therapist and client interactions throughout the entire first psychotherapy session. The results revealed significant effects between the affective exchanges in the therapeutic relationship of both the therapist and the client on clinical outcomes. The researcher employed multivariate statistics with appropriate univariate follow-up procedures to determine group differences between SPAFF codes on the symptom outcome groups and CCAPS subscales. The researcher also used path analysis following Actor Partner Independence Model assumptions (Kenny et al., 2020) to examine the interdependent emotional dynamics between therapist and client on client symptom outcomes, and these revealed significant effects between SPAFF scores observed in the first session on client outcomes at the termination of therapy. The results add to the current research literature, highlighting patterns of significant affective behaviors in the first psychotherapy session on client outcomes. The study reveals that the first session cannot be minimized, as it has a greater impact on client outcomes than previously believed. The conclusion also outlines the study’s implications for clinical practice, graduate training, and research.

Show moreThe purpose of this quantitative, quasi-experimental study was to examine the effects of a standardized case conceptualization training workshop on 104 psychotherapy practitioners recruited from the community. A secondary purpose was to examine the relationship between participants’ attitudes about evidence-based practice and the effects of the training. Participants attended two 3-hour training workshops, which taught the integrative case conceptualization model developed by Sperry (2010b). Pre- and postintervention case conceptualization skills were assessed using the Case Conceptualization Evaluation Form (CCEF) 2.0, an updated version of the instrument used in previous studies. Additionally, participants’ views about case conceptualization were assessed before and after training using the Views about Case Conceptualization (VACC) instrument. Participants’ attitudes about evidence-based practice were also examined as a possible mediating variable between training and effect. These attitudes were assessed using the Evidence-Based Practice Attitudes Scale (EBPAS). Workshops were separated by four weeks in order to assess whether initial training effects persisted over time. Change in case conceptualization skill was analyzed using repeated measures ANOVA. Participants’ mean CCEF 2.0 scores significantly increased (p < .001) from pre-test (M = 11.9; SD = 7.74) to post-test (M = 36.7; SD = 7.80) following the first workshop. The second workshop took place four weeks later with 74 of the original 104 participants. It built on the content of the first workshop and introduced advanced concepts such as client culture, strengths and protective factors, and predictive ability. Participants’ mean CCEF 2.0 scores also significantly increased (p < .001) from pre-test (M = 35.1; SD = 8.11) to post-test (M = 66.3; SD = 10.95) following the second workshop. There was a small but statistically significant (p < .005) decrease of 1.5 points in mean scores from the end of Workshop I to Workshop II, indicating that the effects of the training deteriorate slowly over time. Participants’ attitudes about evidence based practice and some demographic variables were significantly related to training effects. Stepwise hierarchical regression analysis determined that these individual variables account for various portions of the variance in CCEF 2.0 scores. This study’s theoretical, practice, and research implications are discussed in detail.
