Show moreRapid social and economic changes are modifying health care delivery systems and health care financing. These changes require that primary care physicians develop new collaborative approaches to patient care, including a reassessment of relationships with other primary care providers, particularly nurse practitioners. The purpose of this research was to explore and define what constitutes a successful collaborative practice. Open-ended, interactive interviews were conducted with three dyads of physician - nurse practitioner in the ambulatory care setting. Data analysis using the grounded theory method revealed six key elements associated with effective collaborative practice: Communication, competence, autonomy, coordination, trust and personality. This research contributes to clarification of the barriers that hinder a collaborative practice and suggests many benefits derived from such a practice. The findings of the study may inspire other health care providers to enter a collaborative practice to achieve excellence in patient care. Ethical decisions cannot be achieved by a single care provider, but rather require the expertise and unique abilities of both physician and nurse practitioner.

A nurse's story, a reflective remembrance of a nursing situation, was explored for the illumination of nursing's values embedded in the service activities of the nurse. Using qualitative descriptive content analysis, the story was studied for significant statements depicting activities of the nurse and for the values guiding those activities. The analysis revealed three transcendent values, reflected in every activity: Caring, respect for the dignity of the other, and inner harmony. The analysis further revealed eight actualizing values, individually embodying the transcendent values: compassion, competence, courage, humility, honesty, commitment, trust and hope. The wholeness of the inquiry is presented using metaphor to illuminate the meaning of nursing's values in nursing practice.

Six essences of being an RNFA emerged from the data: being a nurse/nursing; a way of being with others/presence; a way of doing for others; constancy/continualness; experience/knowing; and, a sense of self-completeness. These were interwoven against the lifeworld existentials of relationality, spatiality, temporality, and corporeality. The findings revealed that the RNFA offers uniqueness as nursing and uniqueness as experience.

Chi-square analysis was used to test the significance of differences between program completion rates of remediated and nonremediated groups of practical nursing students. Eight null hypotheses were tested. The subjects in this ex post facto study were the 362 practical nursing students who entered the Broward County Practical Nursing Program between April 1988 and August 1989. The subjects included 195 Black females, 138 White females, 11 Black males, and 18 White males. Approximately sixty percent (218) of the students completed the program. Classification accuracy or (hit-rates) for all possible subsets of predictor variables (age, gender, race, ACT, Career Preparation Program sub-test scores, education level, year diploma received, nursing theory score, number of courses completed) were generated for the total sample, completers, and dropouts. The classification accuracy of predictor sets (also called models) were compared to what could be achieved by simply assigning all subjects to the larger group (completers). Analyses revealed that a number of cross-validated models, or predictor sets, were significantly better at predicting success than both maximum and proportional chance criterion. The model that was the best predictor of dropouts contained the variables age, reading sub-test score, math sub-test score, and average nursing theory score. The best predictor set for the total sample contained the variables age, race, nursing theory score, and number of term one courses completed. Significant differences (p < .05) between the program completion rates of LPN students requiring remediation before program entry and those not requiring remediation were found for all sub-tests except reading. These findings add to the growing body of knowledge that may provide program administrators a better understanding of some of the factors affecting attrition from practical nursing programs.

Nurse practitioners, in particular, will be instrumental in filling the gap in primary care availability engendered by the increasing specialization of physician practice and increased access to healthcare made possible by the provisions of the PPACA. The need for this study was identified through gaps in the literature related to nurse practitioner practice; specifically, the paucity of quantitative research regarding patients’ perspectives of core holistic nursing values in nurse practitioner care, and, since nursing care is by definition and tradition holistic in nature, this inquiry attempted to quantify the degree to which nurse practitioner care upholds and preserves core holistic nursing values.

One of the aspects of healthcare hierarchy is the need for nursing care of the patient. Nursing care and observation provide basis for nurses to communicate with other aspects of healthcare system. The ability of capturing and managing nursing practice is essential to the quality of human care. The thesis proposes knowledge based decision making and analyzing system for the nurses to capture and manage the nursing practice. Moreover it allows them to monitor nursing care quality, as well as to test an aspect of an electronic healthcare record for recording and reporting nursing practice. The framework used for this system is based on nursing theory and is coupled with the quantitative analysis of qualitative data. It allows us to quantify the qualitative raw natural nursing language data. The results are summarized in the graph that shows the relative importance of those attributes with respect to each other at different instances of nurse-patient encounter. Research has been conducted by the Department of Computer and Electrical Engineering and Computer Science for the College of Nursing.

Study findings supported role theory assertions that role strain and stress, manifested as role ambiguity, role confusion, role overlapping, and role overload occur when the role transition process and role expectations are not clarified or nurtured among role partners. Implications for nursing practice include the following: (1) Nurses in clinical and administrative practice need to be sensitized to the importance of nurses and ancillary personnel being active participants in the work redesign process, and must support the value of caring that transpires in the nurse-patient relationship. (2) Role theory can be utilized to understand dynamics that occur in work settings of nurses and assistive partners.

Collaboration is a single, temporal event that can occur intermittently in the day to day practice of health care. Nurse-physician collaborative practice is a dynamic process, a commitment to interact on a professional level, that empowers the participants to blend their talent, to achieve a goal that neither can do alone. Despite the fact that collaborative practice demonstration units in the early 1980's showed positive outcomes for patients, nurses, physicians and hospitals, this practice has all but disappeared. This researcher believes this practice modality to be vital in today's health care environment. This study, therefore, addressed the first step in reemergence by asking physicians to define the term, nurse-physician collaborative practice. Over 50% of this select population were able to provide definitions consistent with the health care literature.

Show moreThis study's purpose was to explore, through a descriptive method, the registered nurses' experience of being assisted in practice by multiskilled personnel for the provision of direct patient care. The multiskilled personnel were unlicensed, functioned in an assistive role to the nurse, and were trained at the hospital to perform technical direct patient care tasks. Data sources included interviews with three registered nurses. Study findings supported the existence of a caring nurse-multiskilled assistant relationship that was characterized by the concepts of communication, task responsibility, reciprocal helping, respect, commitment to a shared goal, and nurse leadership. Through a dialectical process, where the thesis was the nurse-patient relationship as identified in the study and the antithesis was the multiskilled assistant-patient relationship, the nurse-multiskilled assistant-patient relationship was synthesized. The study findings in relationship to previous studies, team theory, and Nursing as Caring theory were discussed.

Speak for Myself™ (SFM) was developed for use at the bedside so that communication may be facilitated between patient and nurse when the patient is voiceless. The objective of this study was to evaluate Speak for Myself™ at the bedside and to measure the outcomes. This was a mixed methods, one group pre-test-post-test, quasi-experimental study. Twenty adult patients in three hospitals in South Florida agreed to use Speak for Myself™ during their acute care hospital stay (M = 8.86 hours). This group of participants (n = 20) ranged from 45 to 91 years old (males = 14; females = 6). Of the participants, 15 (75%) self-identified as European American, 2 (10%) self-identified as Hispanic, 2 (10%) self-identified as African American, and 1 (5%) self-identified as Asian. Ten of the participants (50%) were in respiratory failure. Two (10%) were receiving oxygenation measures related to unspecified complications of their illnesses. Of the remaining eight participants (40%), one each was receiving oxygenation measures due to atrial fibrillation, arteriosclerotic heart disease, cardiogenic shock, endocarditis, neck abscess, renal failure, status post seizure activity, and tongue metastasis.

An inhouse, around-the-clock laborist service delivery model has been associated with positive outcomes, but little is known about the laborist structure’s contribution to the labor-anddelivery working environment. The purpose of this descriptive correlational study was to explore the effects of physician service delivery model on safety culture, nurse-physician collaboration, and nurses’ job satisfaction. An additional purpose was to examine associations between nurses’ perceptions of safety culture, nurse-physician collaboration, and job satisfaction. Ray’s (1981, 1989) Theory of Bureaucratic Caring and Homan’s (1974) Social Exchange Theory guided this study. A survey consisting of demographic questions, the Collaborative Practice Scale (Weiss & Davis, 1985), the Hospital Survey on Patient Safety Culture (Agency for Healthcare Research and Quality, 2015; HSOPSC), and the McCloskey and Mueller Satisfaction Scale (McCloskey & Mueller, 1990) was distributed to registered nurses (RNs) nationwide. The results indicated that nurses in facilities using the around-the-clock model had higher perceptions of nursephysician collaboration, but not of safety culture or job satisfaction in relation to the physician service-delivery model. Significant moderate-to-strong correlations between nurses’ perceptions of patient safety and job satisfaction, and a weak correlation between bedside nurses’ perceptions of nurse-physician collaboration and job satisfaction were demonstrated. Additional significant correlations were found between the instrument subscales. Control/responsibility in the MMSS scale was positively associated with both management support for patient safety, supervisors’ and managers’ expectations and actions promoting patient safety, and overall perceptions of safety in the HSOPSC scale. Praise and recognition in the MMSS scale were positively associated with supervisor/manager expectations and actions promoting patient safety in the HSOPSC scale. Further appraisal is needed to understand the mechanism by which the laborist model affects patient care and work environment. Recommendations for future research include replicating the study with a larger sample sizes in specific groups based on the role and scheduled shift, conducting the study in a single system or location to mitigate the effects of other variables; and exploring physicians’ perspectives on the variables being studied.

Show moreThe objective of this study was to describe the lived experience of caring for persons who had a peaceful death in the intensive care units in Thailand. A qualitative research design informed by hermeneutic phenomenology was used to analyze data. Participants were 10 intensive care nurses working at adult intensive care units in south Thailand. A snowball purposive sampling method was used to select the participants. Participant inclusion criteria were at least six months' critical care nursing experience, experience in caring for a person who had peaceful death, able to describe peaceful death, and willing to participate in this study. Participants who met the inclusion criteria were interviewed. Face-to-face individual verbal interviews were conducted in the Thai language. These interviews were audiotape recorded. Descriptions were transcribed and translated for data analysis. Van Manen's (1990) hermeneutic phenomenological approach was used to analyze and interpret the data. The findings of this study were presented in each of 4 categories of the lived world of temporality, of spatiality, of corporeality, and of relationality. The description of the lived experience of caring for persons who had a peaceful death in ICU was, "understanding the other through the valuing of experience and enhancing relations with others by recognizing time is short and is a priority." This study may contribute to nursing knowledge of the end-of-life care to enhance a peaceful death in intensive care units congruently with Thai culture and society. In addition, this study directs the translations of its knowledge into implications that will benefit in helping Thai nursing to move forward. The implications of this study in advance will benefit terminally ill persons and family members regarding receiving good quality end-of-life care.
