Show moreHeart disease remains the number one cause of death in America. The role of primary care in mitigating the prevalence of heart disease is to identify and reduce risk factors. One of the primary risk factors is atherosclerosis which can lead to coronary arterial disease if uncontrolled. A plant based/vegan diet is the safest and one of the most effective modalities for reducing the risk of or even reversing atherosclerosis and associated heart disease. This DNP project sought to assess the effectiveness of a 6-week plant-based diet intervention on cholesterol, perceived wellbeing, and perceived stress on patients diagnosed with hyperlipidemia. After the 6-week intervention period, there was an average 10.2% decrease in total cholesterol, 17.1% drop in LDL cholesterol, and an 8.7% increase in HDL cholesterol. Furthermore, 100% of participants recorded an increased sense of perceived wellbeing and there was an average 44% decrease in self-reported stress levels. The results of this study suggest that the plant-based diet intervention is an effective method for not only lowering cholesterol and the risk for heart disease but for also improving wellbeing and lowering stress. However, further investigation with a larger, more diverse sample is necessary.

Weaver Health Center, Florida. Participants received a pre and posttest using the Beliefs about Medicine Questionnaire (BMQ) and Adherence Starts with Knowledge-12 (ASK-12) questionnaires which helped design, guide, and infer the intervention effects on adherence. The intervention involved; (a) two designed patient educational sessions, 4-weeks apart, on definition of high blood pressure, causes and risk factors for hypertension, sign and symptoms of uncontrolled hypertension, the benefits of taking antihypertensive medications and adhering-aiding tools; (b) self-monitoring utilizing Omron blood pressure machine along with blood pressure and medications logs; and (c) pill-box reminders. Upon completion of this QIP, adherence rates improved by 100% and blood pressure (B/P) improved in three months.

Show moreDue to recent advances in medical technology, the number of children with special healthcare needs (CSHCN) is steadily growing in the United States. CSHCN comprise more than 40% of overall healthcare costs nationwide, even though this group is only comprised of 16% of the U.S. child population. There are significant differences between private and public health plans in terms of cost, adequacy, and parent satisfaction. As an added benefit, some public and private health insurance plans offer nursing care coordination (or nursing case management) services. The purpose of this study was to understand the experiences of parents of CSHCN enrolled in public and private insurance with or without a nurse care coordinator assigned. A phenomenological approach was used. One-to-one semi-structured interviews were conducted with 16 participants. Colaizzi’s (1978) eight steps of data analysis was the selected methodological interpretation. Five themes emerged from this study: Struggle with Self-Preservation, Abandonment and Isolation, Self-Reliance and Advocate, Interdependence, and Lifeline. These study findings highlighted the major differences with parents navigating their child’s health insurance. These differences were dependent on the type of insurance and sources of support available. The experience of parents with a nurse care coordinator differed from parents without a nurse. Parents of CSHCN enrolled in public insurance with a nurse care coordinator considered the nurse to be a primary source of support. These nurses were instrumental in solving problems with the public health plan. On the contrary, parents with CSHCN enrolled in private insurance without a nurse care coordinator carried this additional burden. There were also noticeable differences in parents’ satisfaction with the adequacy and cost of their child’s health insurance. Parents of CSHCN enrolled in private insurance voiced dissatisfaction with higher costs associated with their child’s plan. Meanwhile, parents of CSHCN enrolled in Medicaid expressed that out-of-pocket expenses were covered. Results from this study can be used to make policy changes by insurance companies. Incorporating nursing care coordination not only results in healthcare savings for the health plan but improved health outcomes for its members as well.

Show moreProviding family caregiving during transitional care of a stroke patient is the most challenging for family caregivers. This is because of the shift in the care environment from a hospital with an organizational culture to a home with a traditional culture. Yet, cultural influences on family caregiving during transitional care are not well studied. This ethnographic study aims to describe the family caregiving of older Thai-Isan stroke survivors through the transition from hospital to home. This study employs Lininger's culture care theory (CCT) as the theoretical framework and utilizes the ethnonursing method to discover cultural knowledge. Data was collected through a demographic questionnaire, participant observations, and a semi-structured interview. Data was analyzed using the Four Phases of the Data Analysis Enabler. Fifteen dyads of older stroke patients and their family caregivers were recruited following the inclusion and exclusion criteria. The research settings included Srinagarind Hospital units and key informants’ homes in a Thai-Isan community.

Show moreThe veterans are a unique community shaped by past military experiences that may have positive and negative effects on their well-being as soldiers. With nearly half of the 18.4 million veterans in the United States living beyond 61 years, age-related physical and mental health conditions may impact the veterans’ well-being. Promoting veterans’ well-being has been studied extensively but literature on how they define it is scanty. It is critical to understand how veterans define well-being to inform future program development to address mental health needs. The purpose of this study is to discover the meaning and the lived experience of well-being among veterans. The methodology used for this study was a qualitative hermeneutic interpretative phenomenological approach to understand the meaning of veterans’ well-being. It allowed the phenomena to surface while exposing the essence of and the experience. The sampling method for recruiting participants was purposive sampling, with semi structured interviews conducted via ZOOM and direct observations at Veteran Service Organizations (VSOs) to obtain data. Fourteen veterans participated in this study, comprising eight males and six females with majority aged over 61 years. In terms of race/ethnicity, 64 % were White, 21% Hispanic and 15 % African American. With the use of Max van Manen’s analysis strategy, seven major themes emerged: (a) A Connection With A Special Bond To The Military Sisterhood/Brotherhood, (b) The Lingering Effects Of Military Service Continue To Simmer In Veteran life, (c) Feeling Grateful/Prideful Through Appreciation For Their Military Service, (d) Healthy Companionships/Relationships With Mutual Respect Yield Veteran Equilibrium, (e) Creating A Veteran Framily/Fraternity, (f) Self-fulfilment By Giving Back And Making A Life With A Purpose and (g) Veterans Living A Healthy Life Caring For Self. Six of the seven themes addressed the veterans’ well-being. The lingering effects have persisted and challenged the Veterans’ emotional well-being. However, they were able to express their military camaraderie even in the veterans’ life. Programs based on social relationships that allow connections with veterans and/or their significant others need to be established.
