Show moreThe purpose of this study was to examine nursing's values as they are lived day-to-day in nursing practice. A nurse's story, a reflective remembrance of a nursing situation, was explored for the illumination of nursing's values embedded in the service activities of the nurse. Using qualitative descriptive content analysis, the story was studied for significant statements depicting activities of the nurse and for the values guiding those activities. The analysis revealed three transcendent values, reflected in every activity: Caring, respect for the dignity of the other, and inner harmony. The analysis further revealed eight actualizing values, individually embodying the transcendent values: compassion, competence, courage, humility, honesty, commitment, trust and hope. The wholeness of the inquiry is presented using metaphor to illuminate the meaning of nursing's values in nursing practice.

Show moreThe purpose of this study was to discover the meaning of school body mass index (BMI) screening and referral to parents. The goal of school BMI screening and referral is to provide information to compel parents to change their child's diet and activity levels when overweight and obesity are identified. Measuring BMI in schools and alerting parents to findings above what is considered normal is one intervention to reduce overweight and obesity that has been utilized since 2001 in Florida. To determine the meaning of BMI screening and referral to parents a mixed methods approach was utilized. Voluntary interviews of 20 parents who had received BMI referrals for their children were conducted. Analysis of the interviews was guided by phenomenology, as delineated by van Manen (1997). A cross sectional survey developed by Ruggieri (2012), was distributed to measure parent beliefs and opinions regarding the BMI screening and their re action to referrals. Findings from interviews indicated that parents ascribe different meaning to school BMI screening and referral. Overarching themes of changing and reflecting were discovered. The themes were manifested as dichotomies; some parents reflected on their role as parent and were more satisfied with the process; they reported change of diet and activity for their families upon receipt of a BMI referral. Others reflected on the role of the school in their child's life. They were more dissatisfied with the school screening and recommended changes in the screening and referral process. Suggested changes for school screening and referrals included subthemes: sensitivity, accuracy, privacy, and notification. Parents responded in the survey that they would change diet and activity for their families if they were told by the school that their child had a weight issue. Actions taken upon receipt of a BMI referral were not limited to changing diet and activity levels but also included discussing weight with their child and others. Parents denied they would be offended by a BMI referral. School BMI screening and referral is a valuable and effective intervention to address child overweight and obesity, especially if the process is accomplished with characteristics that parents deem caring.

Heideggerian hermeneutical phenomenology served as both the guiding philosophy and methodology for this research study, while Eriksson’s (1981) theory of caritative caring provided the caring science lens. Diekelmann, Allen, and Tanner’s (1989) seven-stage method of hermeneutical analysis provided the structure for data analysis. The relational themes that were interpreted were: Experiencing the Unimaginable, Awakening to a Changed Reality, Agonizing for Others, Compounding Losses, Finding a Way Forward, and Being Transformed. These six relational themes are illuminated and aesthetically re-presented in six watercolor paintings. The constitutive pattern Suffering With and For Others expressed the meaning of suffering for participants through the 2010 earthquake in Haiti as a lived experience.

Show moreThe purpose of this study was to explore the relationship between green environments and resilience in older adults. It had two aims: 1) to explore the effect of a reflective garden walking program on resilience and three of its related concepts - erceived stress, personal growth initiative, and quality of life - in older adults, and 2) to explore the resilience patterns of older adults engaging in the reflective garden walking intervention. A parallel mixed method design using a quasi-experimental quantitative and a descriptive exploratory qualitative approach was used. Participants engaged in a six week reflective garden walking program. By the end of the program, resilience levels exhibited a slight increase and perceived stress levels a decrease. The qualitative data supported some beneficial effects of the reflective garden walking program, but also indicated that much of the participants' experience of resilience may have been related to the rich social and nature-filled environment in which already they lived. Patterns of resilience that appeared in the data were maintaining a positive attitude, belief in one's self in the face of one's vulnerabilities, woven into the social fabric, purpose and meaning, personal strength, and communities for growing older.

Show moreThe purpose of this qualitative study was to explore the process of complex healthcare decision-making by parents for their children within the milieu of a pediatric palliative care team. As healthcare has advanced, the number of children living with complex chronic conditions has increased. Decision-making by parents for their children referred to palliative care has not, up to this point, been widely explored by nursing. A purposeful sample of 22 participants, parents of children in life-limiting or life-threatening situations were asked: What matters most during the process of complex healthcare decision-making? A constant comparative method was used to analyze data collected from semistructured interviews and the theory of embodying parenting in the midst of conflicting complexity emerged. J. Watson’s (2020) theory of human caring grounded the researcher in living caritas processes throughout the study. All participants experienced a disruption of their expected parenting and family normal. Through constant comparative data analysis, the core category of embodying parenting was identified. Feeling respected as the parent, thoughtfully making right decisions, and maintaining presence support embodying parenting. The basic social process identified was finding a unique path. The process of finding a unique path included connecting with a supportive community, claiming decisional authority, moderating negative thoughts and feelings, and adjusting expectations. Living in peace was achieved by finding a unique path to embodying parenting in the midst of conflicting complexity. For the participants in this study, accepting circumstances, feeling gratitude, and perceiving life differently led to living in peace.

Show moreThe purpose of this descriptive phenomenological study was to give voice to an underserved and vulnerable population in Uganda, Africa to advocate for health priorities. Nineteen members of the village of Namagera, identified as a leper colony, were selected by community members and volunteered to participate in every element of this study. Descriptive phenomenology was the guiding philosophy and community-based participatory research methods and photovoice provided the lens for social action. Leininger’s theory of culture care diversity and universality (Leininger & McFarland, 2006) anchored the study in cultural caring and the community nursing practice model (Parker et al., 2020) kept the researcher grounded in the purpose of advancing the science of caring in community nursing. The research team included the researcher, the 19 participants, and six research assistants, who also served as language facilitators. Using photovoice methods, participants identified strengths and needs and used digital cameras to capture health concerns in the community. Eighty photos were analyzed by the participants and priorities were determined. The selected photos were placed on a photo board and shared with the whole community for input. Community members selected the health priorities and ranked them in importance: access to clean water, sanitation, disease, challenges for the elderly and disabled, lack of medical treatment, transportation, unemployment, and protection of the environment. The participants categorized the health priorities into three clusters of themes: basic needs, safety, and social/environmental. The themes were further incorporated into action plans identifying impeding factors for which the community would require outside assistance and promoting factors for which the community could solve themselves. Findings of this study illuminate the emergence of community empowerment: Community members found their voices, identified health priorities, and advocated for solutions.
