Show moreThe research determined whether hedonistic immersion imagery or epistemic cultural immersion imagery would be more likely to attract college students to book a spring break cruise. Photo Sorts, a projective method, was used to present seven hedonistic and seven epistemic cultural photographs to a convenience sample of college students to determine preferences for Caribbean island excursions. Subjects were asked to assume that they were on a cruise in the Caribbean island; deciding which excursions to take. Forty respondents were tasked with looking at fourteen photographs; placing each one into either the “Do not want to do” or “Want to do” bin. Nominal variables, categories (Want to do/Do not want to do), and an analysis based on counts rather than rating scales characterized the research. Thus, the Chi-Square method found, at 90% confidence, that Royal Caribbean should emphasize hedonistic imagery in its video and digital brochure to attract cruise-bound college students.

Show moreThe overall purpose of this mixed method study was to describe adolescents’ experiences of ease while enduring treatment for cancer. Specifically, the study used avatar image-guided story-sharing (AIGSS) to explore adolescent sense-of-self, experiences of feeling uneasy, descriptions of qualities of ease, and best places for finding ease during treatment. The innovative research approach used in this study incorporated avatar images representing ‘ease’ in typical environments. By testing a developmentally relevant approach, AIGSS, to explore ease for adolescents enduring cancer, this study addressed a pressing need for meaningful approaches to engage adolescents in health-related dialogue about what matters most. While 10 qualities captured adolescent ease (beauty, fun, safety, strength, connectedness, calmness, rest, comfort, independence, and familiarity), connectedness, strength, fun, rest, and safety were the most significant qualities of ease threaded throughout stories. Two themes of self-reflected identity emerged: (a) valued personal qualities (extrinsic/admired) and (b) infused unique demeanors. Three themes described adolescent uneasiness during cancer treatment: (a) persistent uncertainty, (b) fearful anticipation, and (c) disrupted self. While home was most conducive for ease, personal meaning was given to the hospital enabling finding ease. This new knowledge is foundational for nurses who support adolescents during cancer treatment. This research lays the groundwork to re-define ease as a relevant outcome of quality nursing care, focusing on positive outcomes rather than catastrophic ones such as infection and death. Advancing caring science with interactive participative research enhanced communication with adolescents, having implications for both clinical research and pediatric oncology nursing practice.

Show moreThe pioneering work of Ödegaard (1932) was the first to link migration and schizophrenia by reporting rates in Norwegian immigrants in Minnesota as twice that of native Minnesotans and of Norwegians in Norway. However, only in recent decades has an interest in migration and schizophrenia been rekindled as a result of reports of elevated rates of schizophrenia in Afro-Caribbean immigrants in the United Kingdom in the mid- 1960s (Hutchinson & Haasen, 2004). Later studies reported elevated rates in secondgeneration Afro-Caribbean immigrants compared to first-generation (Harrison, Owens, Holton, Neilson, & Boot, 1988). In the United States, Blacks were diagnosed with schizophrenia 2.4 times more often than Whites (Olbert, Nagendra, & Buck, 2018). However, mental health researchers in the United States generally combine all individuals of African descent as African- Americans. This practice obscures the nuances of culture and ethnicity within the Black subgroups as well as the immigrant status of Afro-Caribbeans. This research focused on the Afro-Caribbean immigrants and factors that predict risk for schizophrenia within this population. The process of migration is a complex enterprise that produces stressors and challenges, the effects of which are multifaceted. The social and environmental forces that parallel the process of migration may predispose individuals to severe psychiatric disorders such as schizophrenia. Socio-political dynamics in the host country that marginalize others of different cultural and/or racial persuasions can compound the negative effects of post-migration. Therefore, migration is considered a social determinant of health. Empirical evidence has substantiated that socio-environmental factors such as urbanicity, discrimination or socio-economic deprivation, social support, and goal striving stress are potential contributing factors to the development of psychotic disorders in immigrants. Moreover, evidence has supported that the darker the skin color of the immigrant the greater the risk (Cantor-Graae, 2007). The findings of this study confirmed that for Afro-Caribbean immigrants stressors in the post-migration phase such as discrimination, limited social support, and economic hardship that can be compounded by the number of dependent children were identified as possible predictors of risk for schizophrenia. This risk increased with length of residency and continued into the second-generation.

Show moreBackground: A global cervical cancer health disparity persists despite the demonstrated success of primary and secondary preventive strategies, such as cervical visual inspection (VI). Cervical cancer is the leading cause of cancer incidence and death for women in many low resource areas. The greatest risk is for those who are unable or unwilling to access screening. Barriers include healthcare personnel shortages, cost, transportation, and mistrust of healthcare providers and systems. Using community health workers (CHWs) may overcome these barriers, increase facilitators, and improve participation in screening for women in remote areas with limited access to clinical resources. Aim: To determine whether the accuracy of VI performed by CHWs was comparable to VI by physicians or nurses and to consider the affect components of provider training had on VI accuracy. Methods: A systematic review and quantitative meta-analysis of published literature reporting on VI accuracy, provider type, and training was conducted. Strict inclusion/exclusion criteria, study quality, and publication bias assessments improved rigor and bivariate linear mixed modeling (BLMM) was used to determine the affect of predictors on accuracy. Unconditional and conditional BLMMs, controlling for VI technique, provider type, community, clinical setting, HIV status, and gynecological symptoms were considered. Results: Provider type was a significant predictor of sensitivity (p=.048) in the unconditional VI model. VI performed by CHWs was 15% more sensitive than physicians (p=.014). Provider type was not a significant predictor of accuracy in any other models. Didactic and mentored hours predicted sensitivity in both BLMMs. Quality assurance and use of a training manual predicted specificity in unconditional BLMMs, but was not significant in conditional models. Number of training days, with ≤5 being optimal, predicted sensitivity in both BLMMs and specificity in the unconditional model. Conclusion: Study results suggest that community based cervical cancer screening with VI conducted by CHWs can be as, if not more, accurate than VI performed by licensed providers. Locally based screening programs could increase access to screening for women in remote areas. Collaborative partnerships in “pragmatic solidarity” between healthcare systems, CHWs, and the community could promote participation in screening resulting in decreased cervical cancer incidence and mortality.

Physical and cognitive recovery from brain injury may take several years. Children’s recovery varies, depending on numerous factors including pre-injury conditions and injury severity. While children and families are eager to return home to familiar activities, there are often significant physical, cognitive, behavioral and emotional changes that challenge families. Successful community reintegration depends on the ability of the family to understand and support the child, dealing with and responding effectively to those challenges. The purpose of this study is to understand how parents manage the care and community reintegration of their child who has experienced a TBI over time. This study utilized a mixed methods approach exploring the dimensions of the health challenge faced by parents caring for a child after a TBI, critical turning points as they face health challenges, and approaches for movement toward resolving health challenges. Story theory and story inquiry method were used to gather stories from 10 parents of children who experienced moderate to severe traumatic brain injury between the ages of 12 and 18, and between 2 and 5 years ago. Parents’ perceptions of their child’s quality of life and their ability to manage their child’s health challenge were explored using the Pediatric Quality of Life Inventory and Family Management Measure. Health challenges identified were: living with overwhelming personal upheaval, navigating the unknown, and struggling with how to support independence/dependence. Turning points were chronological or epiphanies. Approaches for movement toward resolving were continuously re-creating a new normal, being fully engaged in meeting the needs of one’s child, and embracing caring relationships to construct the new normal. Qualitative and quantitative data were analyzed to synthesize the findings. Results include a sense of ease in managing the health condition of the child associated with continuously creating a new normal. Healthcare providers can support and strengthen family management of children after TBI by understanding the health challenge, critical turning points and how parents move toward resolving.

The ability to sustain and thrive in this role is essential in repositioning nursing as a knowledgeable discipline actively contributing to the redesign of healthcare. This exploratory descriptive phenomenological study was designed to explore and describe the elements that contribute to the sustainment of CNOs in their practice of nurse executive leadership. Ray’s (1989) Theory of Bureaucratic Caring, Authentic Leadership Theory (Wong & Cummings, 2009), and Resiliency Theory (Earvolino- Ramirez (2007) provided the theoretical lens through which this study was grounded. Semi-structured telephonic interviews were conducted with twenty CNOs all with two consecutive years experience in their current role. Six themes emerged after thorough content analysis which describes the lived experience of sustainment. Each theme was supported by several subthemes. Themes emerged as: Loving the Profession, Having a Broader Impact Reflecting on One’s Own Work, Learning to Manage Conflict, Maintaining Work/Life Balance Working with Supportive Leaders.

This new role positions the masters prepared nurse at the patient's bedside to oversee care coordination and serve as a resource for the clinical nursing team, and to bridge the gaps in health care delivery to better meet the needs of patients in all health care delivery settings. Since this is a new role, there is a paucity of research that has been conducted surrounding these nurses. A phenomenological investigation examined the lived experiences of CNLs® to gain understanding about the meaning of leadership at the point of care and to discover the unique expressions of living caring that CNLs® experience as they embark upon this new role in the acute care hospital setting. Ten CNL® participants were interviewed for this study. Their stories about patient situations and relationships with other disciplines were shared with rich description and emotion. Hermeneutic analysis of the text revealed six essential themes. Six essential themes emerged revealing the essence of leading at the bedside and living caring in the CNL® role: navigating safe passage, pride in making a difference, bringing the bedside point of view, knowing the patient as person, helping nurses to grow, and CNLs® needing to be known, understood and affirmed. Taken as a whole through a synthesis of the themes, the understanding of the meaning of leading to CNLs® includes keeping their patients safe, being proud of their accomplishments and the respect gained from others, as well as being a helper and advocate for other nurses., This is accomplished through their privileged place at the bedside, where they come to know their patients as person and work hand in hand with nursing colleagues. When CNLs® are supported by management, and their roles are planned and understood, they are more fully able to optimally practice and live and grow in caring.

Show moreCreating a safety culture is the focus in the current healthcare environment. An inhouse, around-the-clock laborist service delivery model has been associated with positive outcomes, but little is known about the laborist structure’s contribution to the labor-anddelivery working environment. The purpose of this descriptive correlational study was to explore the effects of physician service delivery model on safety culture, nurse-physician collaboration, and nurses’ job satisfaction. An additional purpose was to examine associations between nurses’ perceptions of safety culture, nurse-physician collaboration, and job satisfaction. Ray’s (1981, 1989) Theory of Bureaucratic Caring and Homan’s (1974) Social Exchange Theory guided this study. A survey consisting of demographic questions, the Collaborative Practice Scale (Weiss & Davis, 1985), the Hospital Survey on Patient Safety Culture (Agency for Healthcare Research and Quality, 2015; HSOPSC), and the McCloskey and Mueller Satisfaction Scale (McCloskey & Mueller, 1990) was distributed to registered nurses (RNs) nationwide. The results indicated that nurses in facilities using the around-the-clock model had higher perceptions of nursephysician collaboration, but not of safety culture or job satisfaction in relation to the physician service-delivery model. Significant moderate-to-strong correlations between nurses’ perceptions of patient safety and job satisfaction, and a weak correlation between bedside nurses’ perceptions of nurse-physician collaboration and job satisfaction were demonstrated. Additional significant correlations were found between the instrument subscales. Control/responsibility in the MMSS scale was positively associated with both management support for patient safety, supervisors’ and managers’ expectations and actions promoting patient safety, and overall perceptions of safety in the HSOPSC scale. Praise and recognition in the MMSS scale were positively associated with supervisor/manager expectations and actions promoting patient safety in the HSOPSC scale. Further appraisal is needed to understand the mechanism by which the laborist model affects patient care and work environment. Recommendations for future research include replicating the study with a larger sample sizes in specific groups based on the role and scheduled shift, conducting the study in a single system or location to mitigate the effects of other variables; and exploring physicians’ perspectives on the variables being studied.

Show moreWorkplace mistreatment (bullying, horizontal violence, and incivility) has been shown to impact nurses’ work satisfaction, job turnover, and physical and mental health. However, there are limited studies that examine its effect on patient outcomes. A correlational descriptive study of 79 acute care nurses was used to test a social justice model for examining the relationship between workplace mistreatment, quantified as threats to dimensions of nurses’ well-being (health, personal security, reasoning, respect, attachment, and self-determination), and nurses’ ability to provide quality patient care. In addition, this study considered the moderating effect of caring work environment among co-workers on nurses’ ability to provide quality patient care in the face of workplace mistreatment. Stories of workplace mistreatment were collected anonymously and analyzed for alignment with threats to six dimensions of well-being. Ability to provide patient care was measured using the Healthcare Productivity Survey and a caring work environment was measured via the Culture of Companionate Love scale. The results demonstrated that threats to all six dimensions of well-being described by Powers and Faden (2006) were expressed in nurses’ stories of workplace mistreatment. Furthermore, 87% reported a decrease in ability to provide patient care after an incident of workplace mistreatment. Yet frequency of threatened dimensions did not have a significant relationship with ability to provide patient care. Moreover, there was a significant moderator effect of the caring work environment on the relationship between number of threatened dimensions of well-being and ability to provide quality patient care. Nurses in high caring environments loss less ability to provide care than nurses in low caring environments when one to three dimensions of well-being were threatened. However, this relationship reversed when four or more dimensions were threatened. Implications include further research on the relationship between workplace mistreatment and nurse well-being and changing practice to include fostering a caring work environment in healthcare facilities.

Speak for Myself™ (SFM) was developed for use at the bedside so that communication may be facilitated between patient and nurse when the patient is voiceless. The objective of this study was to evaluate Speak for Myself™ at the bedside and to measure the outcomes. This was a mixed methods, one group pre-test-post-test, quasi-experimental study. Twenty adult patients in three hospitals in South Florida agreed to use Speak for Myself™ during their acute care hospital stay (M = 8.86 hours). This group of participants (n = 20) ranged from 45 to 91 years old (males = 14; females = 6). Of the participants, 15 (75%) self-identified as European American, 2 (10%) self-identified as Hispanic, 2 (10%) self-identified as African American, and 1 (5%) self-identified as Asian. Ten of the participants (50%) were in respiratory failure. Two (10%) were receiving oxygenation measures related to unspecified complications of their illnesses. Of the remaining eight participants (40%), one each was receiving oxygenation measures due to atrial fibrillation, arteriosclerotic heart disease, cardiogenic shock, endocarditis, neck abscess, renal failure, status post seizure activity, and tongue metastasis.

Show moreThe decision to flee one’s homeland is a complex event that can have a life-long impact. The diaspora of the Cuban people has occurred throughout the United States since 1959. Their stories can shed light on the health challenge of leaving one’s homeland and can contribute to a body of knowledge that can inform nursing and health care. This study presents the qualitative findings from the stories of 13 participants who arrived in the U.S. from Cuba during the Mariel Boatlift of 1980. A story inquiry research design, grounded in the theoretical underpinnings of story theory (Smith & Liehr, 2014), was utilized to explore the dimensions of the health challenge of fleeing one’s homeland, turning points and movement to resolve. Deductive and inductive analysis of the health challenge of fleeing one’s homeland revealed the dimensions of trauma related to the pre-migration, migration, and post migration experience, associated losses, and stigma. The upheaval induced stress in the lives of the participants tested their ability to cope. Managing day-to-day and utilizing internal and external resources, the participants moved to resolve the challenge of fleeing their homeland over time. Many turning points shaped the direction of their experience over decades and contributed to their ability to find meaning by becoming self-sufficient, recreating home, and reconstructing a sense of self-identity. Their unique experiences and stories have provided a voice to empower future studies to expand nursing science, influence empathy and understanding through education, foster awareness in practice, and inspire the potential for policy change for the well-being of refugee populations.

Show moreIn the United States, post European-colonial influence and exposure to substances such as alcohol historically mark the beginning of substance use exposure and involvement among all American Indian and Alaska Native (AI/AN) people. Research findings indicate there are strong interrelationships between the origin and prevalence of substance use and U.S.-European colonization, historical trauma, forced assimilation, cultural losses, and relocation among urban AI/AN people (Burt, 1986; Clinton, Chadwick, & Bahr, 1975; Yuan et al., 2010). In an historical sense, the unsuccessful attempts of relocating AI/AN people to urban areas place the future generations of urban AI/AN youth at risk for substance use, health conditions, and health disparities. Nationally, incident rates of substance use among urban AI/AN populations ages 12 and over is rising steadily, two to three folds higher than other ethnicities in urban areas within states such as Florida. The overall objective of this study is to culturally tailor the evidence-based Lowe (2013) Cherokee Talking Circle Intervention for substance use prevention among Cherokee adolescents to that of an Urban Taking Circle Intervention for use among urban AI/AN adolescents in Florida. Therefore, the feasibility of culturally tailoring this substance use intervention among a southeastern urban AI/AN youth was examined and presented within this dissertation.

While many factors are thought to contribute to the changes that are observed in muscle with aging, one significant, yet modifiable factor is dietary protein intake. This study proposed to improve dietary protein intake in a group of middle-aged women living in South Florida over a 12-week period using individualized per-meal protein prescriptions (1.2 g protein/kg body weight evenly distributed for all three meals) provided by a Registered Dietitian, followed by weekly diet coaching from a Nurse Practitioner (coach). Aims included: (1) An examination of the effect of diet coaching on protein intake, nutrition knowledge, muscle mass, strength, and function and (2) An understanding of how diet coaching sessions help in the process of making dietary changes. A parallel mixed methods research design allowed for a synthesis of inferences from each aim resulting in a meta-inference highlighting interdisciplinary collaboration.

There are many detrimental effects of pain including decreased ability to work, depression, isolation, increased pharmaceutical use, and addiction. Pain, on a worldwide scale, remains ineffectively treated and alternative solutions for managing pain are needed. Pain is conventionally treated with pharmaceuticals, primarily narcotics. Continuation of medications for these painful conditions often causes dependence and addiction. The pain and narcotics cycle contributes to the opioid epidemic. The cost in human lives is immense. Pulsed Electromagnetic Field (PEMF) is a holistic modality used for various ailments. This exploratory descriptive research study focused on the experience of individuals using the Micro-Pulse, PEMF, for pain relief. The mind/body connection was a foundation of the holistic theoretical framework for this study. The theoretical grounding for this study was Watson’s (2018) theory of human caring, which is based on a foundation of holistic healing incorporating mind, emotional body, physical body, and spirit for treatment of pain. Understanding an individual’s experience of pain relief will potentially raise awareness and promote the exploration of holistic therapeutic approaches for patients in pain.

Show moreService learning is a pedagogy designed to teach democratic skills to prepare students to become civically engaged members of society. One of the challenges in the field of service learning is to demonstrate the effectiveness of this pedagogy. Common methodological problems include small sample sizes, difficulty differentiating correlation from causation, self-selection bias, and use of primarily qualitative and experiential outcome measures. The literature review failed to reveal any quantitative scales designed for the health professions. The purpose of this study was to develop and test a quantitative scale to measure service learning outcomes among health professions students, before and after an academic service learning activity. Phase 1 of the study, the development phase, involved an extensive review of the literature to develop the conceptual framework and identify the operational indicators to be measured. Items were adapted from existing scales that were consistent with the personal and social responsibility dimensions and found to possess at least minimally acceptable reliability and validity. Items were adapted to add the health professions perspective which resulted in the preliminary twenty-two item scale, divided into four subscales: Civic Responsibility, Self-Efficacy Toward Service, Civic Participation, and Social Justice Attitudes. Psychometric testing of this preliminary PSR Scale was done in 3 studies. Study 1 involved evaluation of content validity with subject matter experts utilizing a Content Validity Index. The scale was modified based on the results of the CVI and recommendations of the subject matter experts. The S-CVI/Ave for the entire scale was .84 suggesting content validity of the PSR Scale. Study 2 utilized principal components analysis of the subscales to validate the dimensions and operational indicators. Data was constrained to four factors which accounted for 60.56% of the total variance. Items with factor loadings less than 0.4 were deleted. Cronbach’s alpha coefficient was calculated for internal consistency. Based on these results, the scale was further revised by deleting items that decreased the Cronbach’s alpha. This resulted in a 16 item scale, containing four subscales, each with four items. The Cronbach’s alpha for the entire revised scale PSR Scale was .94. Study 3 involved testing the final 16 item version for sensitivity. Wilcoxon signed rank analysis revealed statistically significant changes pre and post service learning activity in the Civic Participation Subscale. Civic Participation Subscale items that were significant included “volunteering time to support my community”, “being involved in programs and activities that improve my community”, and “being involved in activities that improve the health of my community”. These findings suggest that participation in a service learning activity can increase civic participation. Psychometric testing of the Personal and Social Responsibility Scale (PSR) support preliminary validity, reliability and sensitivity of the instrument and the premise, consistent with prior research that changes in civic participation can occur as a result of service learning.

The age-related disease of Alzheimer’s threatens the health of older Appalachians, yet research on Alzheimer’s disease (AD) in this population is scarce. In order to improve screening rates for cognitive impairment, Appalachians need to understand their vulnerability. The first step would be to assess their knowledge about AD but a suitable AD knowledge test has not been developed. The purpose of this study was to test the reliability and validity of a new measure of knowledge about AD that is culturally congruent, and to examine factors that may predict AD knowledge in this rural population. A correlational descriptive study was conducted with 240 participants from four samples of older adults in south central rural Appalachian West Virginia using surveys and face-to-face interviews. Results from tests for stability, reliability including Rasch modeling, discrimination and point biserial indices, and concurrent, divergent, and construct validity were favorable. Findings were that although more diversity in test item difficulty is needed, the test discriminated well between persons with higher and lower levels of education [F(2, 226) = 170.51, p = .001]. Using multiple regression, the predictors of AD knowledge included caregiver status, miles from a healthcare provider, gender, and education; (R2=.05, F(4,187) = 2.65, p =. 04). Only years of education accounted for a significant proportion of unique variance in predicting the total BKAD score (t = 2.14, p =. 03). Implications include the need for further tool refinement, testing for health literacy, coordination with recent statewide efforts to educate the public regarding AD, and community based participatory research in designing culturally effective education programs that will ultimately increase screening and detection of Alzheimer’s disease in rural populations.

Show moreThe purpose of this randomized control trial (RCT) was to investigate the effects of a story-sharing intervention on older adults transitioning to long-term care (LTC). The specific aims were (1) to determine the effects of story sharing on the health transition outcomes of depression and well-being of adults transitioning to LTC; and (2) to determine if the sociodemographic characteristics (age, gender, ethnicity, marital status, level of education (LOE), months living at LTC facility, choice to move, and health problem(s) that may have resulted in the move) predict depression and/or well-being. Story sharing was defined as the respectful space where one tells and listens to stories of others (Heliker, 2009) while being guided by another (the investigator). Meleis’ middlerange theory of transitions (MMRTT) (Meleis et al., 2000) was used as a guide to understand the transitioning process. A convenience sampling design was used to recruit 100 participants from 11 LTC facilities in Broward County, located in Southeast Florida. The sample included adults, age 65 years and older, who were transitioning to a LTC facility within the past two years. Participants were randomly assigned to the control group (n = 52), who received standard care, or to the intervention group (n = 41), who received story-sharing and standard care. The results indicated there was no significant greater improvement to suggest an Intervention and Time effect for depression and/or well-being. Overall, predictive ability of the sociodemographic variables for depression and well-being were not statistically significant. However, LOE (junior college) did account for a significant portion of unique variance for increased depression, and time, marital status, ethnicity, LOE, and choice to move did account for a significant proportion of unique variance for well-being. Months living in LTC (more months) and marital status (all but divorced) were significant for improved well-being while ethnicity (Hispanic), LOE (high school and junior college), and choice to move (no choice) were significant for reduced well-being. Similar studies using a larger sample size, including non-English speaking participants; lengthier storysharing sessions; and measuring for pain, health, and bereavement may offer additional insights to healthy transition outcomes for this population.
