Show moreObjectives: To describe the co-occurrence of substance use and sexual activity behaviors among Afro-Caribbean adolescents living in South Florida, with attention to legal status, socio-demographic factors and risk-taking attitudes and behaviors. Methods: Convenience sampling was used to select 106 Afro-Caribbean adolescents from community centers in South Florida. A descriptive exploratory study was conducted. Data was analyzed using an independent t test, frequencies and crosstabs. Results: The study consists of 106 adolescents, 75% (n = 79) documented and 25% (n = 27) undocumented. Forty-one documented and 10 undocumented adolescents were sexually active. Of those, 14.6% of the documented and 40% of the undocumented adolescents had been drinking alcohol while engaging in sexual activity; 7.3% of documented and 30% of undocumented adolescents used drugs while engaging in sexual activity. Undocumented adolescents had less adult presence before and after school; Creole was spoken at home more than English, and none of the parents had gone to or graduated from college. There was no significant difference in risk-taking and social adaptation scores as measured by the Adolescent Risk-Taking Instrument (ARTI) for the documented and undocumented Afro-Caribbean adolescents. The ARTI had acceptable internal consistency reliability for the risk-taking (.87) and social adaption (.82) scale in this population. The mean score of risk behavior was 2.04 (SD = .44) for documented Afro-Caribbean adolescents and 1.89 (SD = .47) for undocumented adolescents. For social adaptation, mean scores were 3.23 (SD = .45) for documented and 3.20 (SD = .35) for undocumented adolescents. The co-occurrence of substance use and sexual activity is nearly triple for alcohol use and more than triple for drug use when comparing undocumented to documented adolescents., However, scores on the ARTI did not differ.Socio-demographic factors related to risky behaviors suggest that the undocumented adolescents were more at risk. Health risk of undocumented adolescents demands more research attention if nurses wish to address the unique needs of this population.

A qualitative research design informed by hermeneutic phenomenology was used to analyze data. Participants were 10 intensive care nurses working at adult intensive care units in south Thailand. A snowball purposive sampling method was used to select the participants. Participant inclusion criteria were at least six months' critical care nursing experience, experience in caring for a person who had peaceful death, able to describe peaceful death, and willing to participate in this study. Participants who met the inclusion criteria were interviewed. Face-to-face individual verbal interviews were conducted in the Thai language. These interviews were audiotape recorded. Descriptions were transcribed and translated for data analysis. Van Manen's (1990) hermeneutic phenomenological approach was used to analyze and interpret the data. The findings of this study were presented in each of 4 categories of the lived world of temporality, of spatiality, of corporeality, and of relationality. The description of the lived experience of caring for persons who had a peaceful death in ICU was, "understanding the other through the valuing of experience and enhancing relations with others by recognizing time is short and is a priority." This study may contribute to nursing knowledge of the end-of-life care to enhance a peaceful death in intensive care units congruently with Thai culture and society. In addition, this study directs the translations of its knowledge into implications that will benefit in helping Thai nursing to move forward. The implications of this study in advance will benefit terminally ill persons and family members regarding receiving good quality end-of-life care.

A sequential mixed-methods approach that combined quantitative and qualitative data collection techniques was implemented using a pre-experimental, post-test only design with non-equivalent groups to determine differences between the Traditional Model (TM) and Clinical Scholar Model (CSM) in clinical nursing education... Limitations of this study include low sample size and are partially due to limitations of the class size and low survey participation. Additionally, only CSM students attended a focus group, which prevented comparisons of qualitative feedback between groups. Even with these limitations, CSM students scored as well or better than TM students, indicating that the CSM could be a viable model for nursing clinical education.

The purpose of this study was to explore the lived experience of individuals who self-identify as sex addicts. Through snowball sampling, five men and five women between 27 to 45 years old, and older. participated in the phenomenological study. Meaning units and themes were revealed through the participants' experiences as follows: A Connecting with Others: 1. Reaching Out 2. Seeking Shared Understanding 3. Connecting with Your Higher Power B Managing Stigma: 1. Revealing Concealing C Integrating the Past for Recovery: 1. Reflecting Triggers 2. Overcoming Powerlessness. D Being Vigilant: 1. Intentional Refocusing 2. Living an Honest Life E Giving of Oneself: 1. Informing Others 2. Doing Service. The overall structure synthesized from the meaning units and themes was: "The lived experience of seeking health in recovery from sex addiction is dancing on the outer circle, connected to a community that understands fear, shame and the struggle to remain vigilant for pitfalls while intentionally refocusing on living an honest life of giving and receiving." Seeking health incorporates a holistic, community involved, multifaceted approach to recovery. Understanding how individuals seek health in recovery provides a framework to impart meaningful, sensitive nursing care.

Show moreThe veterans are a unique community shaped by past military experiences that may have positive and negative effects on their well-being as soldiers. With nearly half of the 18.4 million veterans in the United States living beyond 61 years, age-related physical and mental health conditions may impact the veterans’ well-being. Promoting veterans’ well-being has been studied extensively but literature on how they define it is scanty. It is critical to understand how veterans define well-being to inform future program development to address mental health needs. The purpose of this study is to discover the meaning and the lived experience of well-being among veterans. The methodology used for this study was a qualitative hermeneutic interpretative phenomenological approach to understand the meaning of veterans’ well-being. It allowed the phenomena to surface while exposing the essence of and the experience. The sampling method for recruiting participants was purposive sampling, with semi structured interviews conducted via ZOOM and direct observations at Veteran Service Organizations (VSOs) to obtain data. Fourteen veterans participated in this study, comprising eight males and six females with majority aged over 61 years. In terms of race/ethnicity, 64 % were White, 21% Hispanic and 15 % African American. With the use of Max van Manen’s analysis strategy, seven major themes emerged: (a) A Connection With A Special Bond To The Military Sisterhood/Brotherhood, (b) The Lingering Effects Of Military Service Continue To Simmer In Veteran life, (c) Feeling Grateful/Prideful Through Appreciation For Their Military Service, (d) Healthy Companionships/Relationships With Mutual Respect Yield Veteran Equilibrium, (e) Creating A Veteran Framily/Fraternity, (f) Self-fulfilment By Giving Back And Making A Life With A Purpose and (g) Veterans Living A Healthy Life Caring For Self. Six of the seven themes addressed the veterans’ well-being. The lingering effects have persisted and challenged the Veterans’ emotional well-being. However, they were able to express their military camaraderie even in the veterans’ life. Programs based on social relationships that allow connections with veterans and/or their significant others need to be established.

Currently, there is no cure for the rapidly growing burden, but there are pharmacological treatments to manage the symptoms, which lead to numerous side effects (Kales et al., 2015). Method: We tested the effectiveness of a non-pharmacological toy interactive pet (TIP) in improving mood/behavior and cognition among twelve persons with mild-severe dementia attending an adult day center (ADC) over 12 visits. Mood/behavioral symptoms were assessed using the Alzheimer’s Disease and Related Dementias Mood Scale (AD-RD), Observed Emotion Rating Scale (OERS), and the Cornell Scale for Depression in Dementia (CSDD). Cognition was assessed via Mini Mental State Examination (MMSE). Paired-sample t-tests, Pearson’s correlation, repeated measures t-test, and a post-intervention qualitative inquiry were used to examine the significance of TIP. Results: All mood scores improved over time, with two showing significance: OERS (M = 73.7/SD=9.6); conditions t(11) = -19.18, p<.001, and CSDD (M = 8.8/SD = 7.2); conditions t(11) = 4.12, p=.002. Over half (0.67%) scored higher on the MMSE post-test than the pre-test: M =10.7(SD = 5.5) and M =12.2(SD=7.1), respectively. Participants stroked and spoke often to their pets. Several family members reported participants sleeping with their pet following the program’s conclusion.

Show moreTransitions in care have been a national priority for decades to promote the safe and timely passage of patients between levels of healthcare and across care settings. Improving hospital discharge processes is an important quality improvement initiative to reduce adverse outcomes, and thirty-day readmission rates and increase patients’ satisfaction after discharge. This qualitative design study explored the perceptions, experiences, and gaps in understanding the needs of the resident and family in the transition from a hospital to a skilled nursing facility (SNF). It is essential to understand the perspective of the residents, families, and healthcare professionals who experience this transitional process to inform future research, and practice initiatives and to effectively prepare the residents and families for this transition. The purposive sample included 15 residents, 13 family members, and 12 SNF staff. The setting was a SNF within a continuing care residential community with an 89-licensed-bed capacity in Palm Beach County, FL. This doctoral research applied a descriptive qualitative methodology to capture the essence of the perceptions of residents, families, and staff participants transitioning from acute care to a skilled nursing facility. Interviews were qualitatively analyzed with an interactive model utilizing open, pattern, and thematic coding, and data (network) display to help organize, inform, and justify conclusions and explanations. Overall, 53% of residents, and 62% of families expressed confusion, frustration, concern, uncertainty, disappointment, and stress.

Show moreRural West Virginia has a very high percentage of older adults. The age-related disease of Alzheimer’s threatens the health of older Appalachians, yet research on Alzheimer’s disease (AD) in this population is scarce. In order to improve screening rates for cognitive impairment, Appalachians need to understand their vulnerability. The first step would be to assess their knowledge about AD but a suitable AD knowledge test has not been developed. The purpose of this study was to test the reliability and validity of a new measure of knowledge about AD that is culturally congruent, and to examine factors that may predict AD knowledge in this rural population. A correlational descriptive study was conducted with 240 participants from four samples of older adults in south central rural Appalachian West Virginia using surveys and face-to-face interviews. Results from tests for stability, reliability including Rasch modeling, discrimination and point biserial indices, and concurrent, divergent, and construct validity were favorable. Findings were that although more diversity in test item difficulty is needed, the test discriminated well between persons with higher and lower levels of education [F(2, 226) = 170.51, p = .001]. Using multiple regression, the predictors of AD knowledge included caregiver status, miles from a healthcare provider, gender, and education; (R2=.05, F(4,187) = 2.65, p =. 04). Only years of education accounted for a significant proportion of unique variance in predicting the total BKAD score (t = 2.14, p =. 03). Implications include the need for further tool refinement, testing for health literacy, coordination with recent statewide efforts to educate the public regarding AD, and community based participatory research in designing culturally effective education programs that will ultimately increase screening and detection of Alzheimer’s disease in rural populations.
